PLAN FOR ACHIEVING SELF SUPPORT
August 1, 1995

Name: Allen

SSN:

1. My work goal is: Office Courier

2. I want my plan to begin: August 1, 1995

I expect to reach my goal by: August 1, 1998

Note: I believe that it will take 3 years for me to become an Office Courier, this PASS is only outlining the first 18 month period of that goal (and only requesting approval for the first 18 months).

3. I will have the following expenses, during the first 18 months, in order to reach my goal:

See enclosure # 1, VCR, TV and educational training tapes for home study courses for computer skills Required for goal achievement:
8/1/95-2/1/97 $50.00/mo. $900.00/total

See enclosure #2, Computer, Multi Station Network, Software, & Bar Code Tracking Technology (including finance charges, set up and installation) Required for goal achievement:
8/1/95-2/1/97 $750.00/mo. $13,500.00/total

Initial Professional Work Clothing Required for goal achievement: 8/1/95-2/1/97 $60.00/mo. $1080.00/total

Meals at Work: $3.00 per day Required for goal achievement: 8/1/95-2/1/97 $60.00/mo. $1080.00/total

TOTAL 18 months, $920.00/mo.$16,560.00/total

(Note: total cost over three years is estimated at $15,000.00 to $20,000.00)

NOTE: The above expenses are shown evenly amortized over the first 18 months. I anticipate that the actual monthly expenses will not reflect the "even amortization" shown above but will vary based on the billing cycles and amounts associated with the specific expense. The final totals for each item are believed to be accurate estimates at this point.

4. I already have the following money or property that I will use to reach this goal:

5. I expect to receive the following income that I will use to reach the goal:
$813.00 wages per month
$565.00 SSDI per month
Note: I will deposit $920.00 per month from the above income in my PASS account.

6. I will keep the money I set aside under my plan in the following bank account: A separate checking account will be set up upon approval of this PASS.

Enclosure #1:VCR & audio cassette recorder for educational training tapes and home study courses for computer skills & Electronic Spelling & Printing Calculator.

I will be using video tapes for self paced training on my computer. I believe this media to be very useful for my learning processes, and will benefit significantly from self paced learning.

Enclosure #2: Computer, Multi Station Network, Software, & Bar Code Tracking Technology.

I am currently working as an entry level, assistant fax and office courier for ALCO Management Services, 1700 Lincoln street, Denver, CO 80203. My position is to assist with very timely fax delivery service for a law firm in Denver and I am partially responsible for guaranteed 15 minute fax delivery on floors 41 through 46 at 1700 Lincoln Street. A significant problem area (or opportunity for improvement) is in the "timely" delivery of faxes and also clear understanding by the recipients of the receipt or non receipt of faxes. Often mis-communications occur as to the actual delivery of important faxes and or location of the faxes in the multi floor building. This Computer and multi station network is as proposed joint solution with my employer that will clearly log, track, and identify the location of faxes and improve a critical component of my job.

My employer has been involved in the development and planning of this solution and is supportive of it's intended results. I believe that through the use of this system my work performance will improve substantially and once the initial phases of the system are installed will clearly place me above SGA and I will be able to eliminate my reliance on SSDI as my work performance increases. The system proposed involves one conservatively priced "server" computer station on the 44th floor collocated with the central fax equipment and 7 additional monitors and keyboards only (not full CPU's) operating as a small Local Area Network (LAN) system. One additional notebook computer will be networked for mobile and special project applications, which I will also use at home to improve my office computer skills.

Bar Code Tracking Technology will be integrated into the system (similar to the technology used by Federal Express) to clearly locate & log all faxes within the multi floor office complex. All system pricing will be conservatively & competitively bid. The projections shown are estimates based on my employer's expertise and input from the planning meetings held to date.

The proposed system is not a requirement of the job but rather is a unique and thoughtful application of universal assistive technology designed to offset the effects of my disability in my work environment. My employer is under no obligation to provide such a system as co workers without disabilities do not require such technology. I am capable of doing the job without the proposed system, but my efficiency is reduced and therefore I perform currently at a considerably less efficient rate with lower quality and reliability. The proposed system is an enhancement similar to a college education to increase employment skills and opportunities.
